Patient Rights

• Receive a copy of the “Patient Bill of Rights and Responsibilities”.

• To know the price of services and procedure.
• To be informed of continuing health care requirements.
• To know the rules and policies that applies to obtaining benefits services.

Patient Responsibility

• You are responsible for following the instructions and medical orders of your treating team and to tell them if you are unable or are not willing to follow the treatment plan.
• You are responsible for the consequences of refusing the treatment. And sign consent for that.
